Referrals

Referrals are sent to the service by general practitioners (GPs) or other health specialists.

Referrals must come through the Community Regional Referral Centre.

Please note: Referrals for patients with diabetes should be sent to the Regional Diabetes Service.

Unless previously arranged with the occupational therapist, no self-referrals are accepted.
There must be physical impairment that is impacting upon everyday function.
Referrals must come through the Community Regional Referral Centre.

Referrals

During the trial phase clients must be referred by their Work and Income case manager. 

The service is being evaluated by a PhD Student using a randomised controlled trial method and needs to be considered with referrals at this time.

Evaluation of the trial should be available early 2019.

You can access the service via your midwife, doctor, Plunket/Tamariki Ora nurse, Whanau Ora, Pasifika provider or Maternal/Baby hub. They can refer you.

Referrals

 Referrals are accepted from a variety of sources including:

Self-referrals including family & friends with client’s consent
Primary health providers - general practitioners (GPs), Plunket nurses, midwives
Secondary health providers - obstetricians, ante/post-natal services, Mothercraft and New Born Unit
All referrals to the Waikato DHB Perinatal Mental Health Service will be triaged by the Adult Mental Health Triage team who will make contact with the client to complete a telephone triage…
