District nursing

District nurses are members of Family Health Teams that provide a wide range of services in their local communities.

What we offer

District Nursing
Expert assessment, management, support and education for:

  • wound care
  • IV management
  • palliative/end of life care
  • ostomy care
  • continence assessment/catheter management
  • assessment for acute personal care and home help assessment (non-ACC)
  • support with home oxygen
  • health education
  • liaison with other health professionals.

Referral criteria

  • All referrals must come through the Regional Referral Centre - Fax  0800 867 333.
  • Self referrals are not accepted. Referral is through your GP or hospital service.

If we are unable to visit

Occasionally we may need to cancel your planned visit.  We will contact you and arrange a new time. (We will not cancel if it could compromise your care or recovery).

8am - 4.30pm Monday - Sunday

On-call service

Limited service available for patients identified by the district nurse after hours and on weekends.

Visiting requirements (for our safety)

Please:

  • restrain and secure dogs
  • do not smoke during  our visits
  • advise us if there are any hazards that may be around your home or workplace.

Cancellations

If you need to cancel a visit please ring your local district nurse base to have your appointment rescheduled.

We are here to help support you to achieve your health goals.

Four things that you can do to get the best possible outcome:
  1. Let us know what your goals for treatment are
  2. Actively negotiate the plan of care with your nurse
  3. Stick to the plan as much as possible, (e.g. resting, diet, wound care instructions)
  4. Let us know if things change.

Wound care

If we are visiting you for wound care, here are three useful things to know:
  • most wounds heal best when not constantly disturbed so you may have a dressing in place for several days
  • please leave your dressing in place between visits unless advised otherwise by your nurse
  • if your wound becomes red, swollen, more painful or has an offensive smell or discharge, please contact your district nurse or doctor promptly.
